GUIDELINES FOR OATA COLLEGE/UNIVERSITY SCHOLARSHIP

  I.          Recommendation Criteria
 
A.        The applicant must be currently enrolled as a full time student in an Ohio college/university pursuing a degree with the intention of becoming an Ohio Licensed Athletic Trainer of the OTPTAT Board and a member of the OATA. More than one applicant from the same college/university may apply for this scholarship.
 

B.         The applicant must currently be either a junior or senior in good standing in that college/university.
    C.        The applicant must have a minimum of two years experience under the supervision of an Ohio Licensed Athletic Trainer of the OTPTAT Board.
    D.        The applicant must have a grade point average of 3.0 on a 4.0 scale of “B” or better.
    E.         The applicant must be sponsored by an OATA member and either:
      1)         Category 1 – OHIO LICENSED ATHLETIC TRAINER OF THE OTPTAT BOARD
      2)         Category 2 – OHIO LICENSED ATHLETIC TRAINER OF THE OTPTAT BOARD/BOC CERTIFIED ATHLETIC TRAINER
       
  II.         Application Process
   
A.        An application may be obtained from the OATA Scholarship Committee Chair or downloaded from the OATA website
    B.         The following documentation must be submitted with a completed application form:
      1)         A letter of recommendation from the sponsor. A second letter of recommendation may come from a coach, professor, physician or other supervising athletic trainer. Each letter will include references to the applicant’s character, work habits, attitude, professionalism and contribution to the college program. These letters would also make reference to the applicant’s aptitude for and interest in the profession.
      2)         An official transcript of all grades earned in college/university.
    C.        Application deadline – Postmarked on or before February 1st R12/08/02, R12/07/03.
    D.        Current application and rating form are located on the OATA web site.  R12/0504.
    E.         One scholarship will be awarded in the amount of $1500.00. It will be awarded for one year only, and it will be non-renewable. R12/08/02.
    F.         The winner’s selection will be based upon merit, which includes: academic achievement, experience in athletic training, professional qualification and letters of recommendation.
